ID card or passport
Biometric passport photo
First aid certificate
Medical examination according to Annex 5 No. 1 FeV
Ophthalmologist’s report according to Annex 6 FeV
performance test according to Annex 5 No. 2 FeV
Certificate of good conduct (Führungszeugnis, type O)

In Bochum the certificate of good conduct may be no older than 3 months. If you drive commercially, you also need the professional driver qualification (BKF).
